Staff record 2018/19 - Template exit survey

The template exit survey has been designed to assist Staff record contacts with the collection of staff leaver information and share good practice. A number of HE providers supplied HESA with their in-house exit surveys, which were used to create the template exit survey.
This template exit survey is provided for guidance only. There is no obligation for this survey to be used by HE providers. HE providers are free to alter this survey to fit their own requirements. Only the 'Activity after leaving' and 'Location (of employment) after leaving' questions are HESA required questions. The survey is provided in three file formats: word document, PDF and .json.
The .json file is a BOS survey file. This file can be imported into the BOS survey system.
